The Adult Patient Assessment Tool
The Adult Patient Assessment Tool (APAT) is a multidisciplinary admission and discharge form utilised within the medical and surgical wards of 20 hospitals in rural New South Wales. It provides for the implementation of
appropriate care, through identification of risks, implementation of strategies, and referral to services for further assessment and intervention. It facilitates the assessment of clinical, social and emotional needs of patients, and is integral in planning and coordinating care across the continuum. It was developed using evidence based practice, and supports State Health guidelines and circulars relating to patient assessment and care. The Care Plan compliments the assessment tool enabling staff to document interventions and care on a daily basis or when a patient's condition changes.
